Incident Reports

Any time an incident occurs at a campus library that requires contacting emergency services or security, the dean should be notified immediately, and an incident report completed as soon as possible. Incident Report Forms can be found on the share drive. The Incident Report should be detailed but not wordy and should include information including all who witnessed the event, time, date, details of what occurred, and the steps taken by the staff members during and following the event.
